Payment Policy
Self-Pay
Concierge medicine is a self pay healthcare model. This means you pay for a membership to retain your own personal practitioner. You can pay either monthly or yearly. Cost for a single membership is $125 a month or $1500 yearly. Family plans start at $200 for two members, with +$10/child/month up to age 18 years old for over 2 members. After age 18 cost will be +$65 for every member who is 18 years old or older for over 2 members. Often people end up paying more due to copays, unneeded exams, tests, etc., and end up paying high deductibles which add up to more than what concierge medicine costs. Many people do not want to wait in a waiting room with other people who are sick or frankly do not have the time to wait. At Black Bag Concierge Medicine you do not have to wait in a waiting room as our providers will see you in the comfort of your own home or via telemedicine visit.

We do not accept insurance but we can provide you a superbill for potential reimbursement from your carrier and for tax purposes. You can also pay your concierge fees with your Health Savings Account (HSA).
About The Owner
Selwyn Layton
My name is Selwyn Layton and I am the CEO and founder of Black Bag Concierge Medicine. As a Family Nurse Practitioner, I have been trained to treat the body as a whole "mind, body, and overall wellness" through preventative care and top-quality treatments. I am able to assess, diagnose, and treat the majority of conditions that I am presented with.

I have gravitated my interest toward mobile health care services for those that want or need to be treated in the privacy of their homes without the wait times in the lobby of a medical clinic. Getting to know my patients and making sure they have quality health care is my top priority as I feel it's important in order to provide them the best and most appropriate care for their medical needs. I also understand that patients with complex conditions may need to be seen by a specialist which I am able to refer them to.

I have been in healthcare over 20 years in different capacities, including being a CNA in a local home health company, a medical surgical nurse, a cardiovascular nurse, an emergency nurse, a rural ground LifeFlight nurse, and a hospital house supervisor. I received my Bachelor's of Nursing from Touro University-Nevada and my first Master's in Nursing Administration from Touro University-Nevada. I became a Family Nurse Practitioner in October 2018 with a Master's of Science in Nursing from Spring Arbor University and received my Doctor of Nursing Practice from Capella University.

I was born and raised in a small ranching/mining town in northern Nevada called Battle Mountain. In 1996 I moved to Cedar City, Utah and attended Southern Utah University (SUU). I married my beautiful wife, Kara Layton, who was raised in Cedar City and together we started our family, adopting two wonderful kids along our life journey.
